Beloved father of Sean (Mel) Forman of Hamilton Township, Ohio, Charles Scott (Karyn) Forman of Goshen, Ohio and Amanda Walker of Goshen, Ohio; loving grandfather of Nick, Samantha, Morgan, Chase, Meaghan, Tate and Danielle; cherished son of Mabel Forman of Hamilton Township, Ohio; caring brother of Lawrence "Sonny" (Pam) Forman of Goshen, Ohio and Helen (Rick) Myers of Bethel, Ohio; loving companion of Sharon Greene of Decatur, Ohio; also survived by a host of additional nieces, nephews, family and friends. Preceded in death by his father, Lawrence E. "Buster" Forman. Entered into rest Wednesday, August 1, 2012 at the age of 64. Mr. Forman was a lifetime farmer. Friends will be received 6-8 PM Sunday, August 5, 2012 at Tufts Schildmeyer Funeral Home, 1668 St. Rt. 28, Goshen, Ohio 45122. Funeral service to be held 10 AM Monday, August 6, 2012 at Tufts Schildmeyer Funeral Home, Goshen, Ohio. Interment Murdoch Cemetery, Hamilton Township, Ohio.
